Medicare Annual Wellness Visits in Vero Beach

PulsePoint Primary Health is an adult (18+) primary care practice in Vero Beach that accepts Medicare.

If you have Medicare Part B, you can use a yearly Annual Wellness Visit, or your one-time Welcome to Medicare visit, to sit down with a primary care clinician and plan your prevention for the year ahead.

Sara Poslaiko, DNP, APRN, FNP-BC provides primary care for adults at PulsePoint. Visits are by appointment, Monday–Friday.

What is a Medicare Annual Wellness Visit?

Medicare describes the yearly Wellness visit as a conversation-based visit to create or update a personal prevention plan, based on your current health and risk factors. It is not a routine physical exam, and it isn't a sick visit.

At a typical Annual Wellness Visit, you can expect to: fill out a short health questionnaire (Medicare calls it a Health Risk Assessment); have routine measurements taken, like height, weight and blood pressure; review your medical and family history; go over your current prescriptions; get a written checklist of the screenings and preventive services that apply to you; have a brief memory and thinking check; talk about advance directives, if you'd like.

Your clinician will explain what applies to your situation. If you have a specific health concern, we may schedule a separate visit so your wellness visit can stay focused on prevention.

Turning 65 or new to Medicare? Ask for a Welcome to Medicare visit

Medicare Part B covers one Welcome to Medicare preventive visit within the first 12 months you have Part B. It's a one-time check-in to review your medical and social history and talk about the preventive services that make sense for you. Like the yearly wellness visit, it isn't a comprehensive physical exam.

During this visit, your clinician will typically review your history, check your body mass index (BMI), do a simple vision test, review your risk for depression, and give you a written plan for recommended screenings.

When you call, tell us you'd like to schedule your Welcome to Medicare visit by name. Please bring: your Medicare card and photo ID; your medical records, if you have them from a previous doctor; your family health history; a list of every prescription, over-the-counter medicine, vitamin and supplement you take, with how often you take each one.

After that, your first yearly Annual Wellness Visit can be scheduled once 12 months have passed since your Welcome to Medicare visit (or since you started Part B). After that, it's once every 12 months.

Medicare open enrollment: check your doctor

Medicare open enrollment runs October 15 through December 7. If you're reviewing plans, check that your primary care practice is in the plan's network before you choose. Medicare Advantage plans each have their own networks. PulsePoint can't recommend a plan, but we're happy to tell you whether we work with a plan you're considering. Call (772) 365-4722. For plan comparisons, visit medicare.gov or call 1-800-MEDICARE.

What about costs?

Medicare explains what it covers for these preventive visits at medicare.gov. If other tests or services are done during the same visit, or if you'd also like a routine physical exam, other costs may apply. Ask our team about costs when you schedule.

Annual Wellness Visit vs. annual physical

An Annual Wellness Visit is a prevention-planning conversation. A routine physical exam is a different visit, and Medicare may not cover it. PulsePoint offers both for adults, so tell us which one you're looking for when you call, and we'll help you schedule the right visit.
